GigCare Health Insurance: What It Is, How It Works, and Who It's For (2026 Guide)

A neutral, broker-level explanation of GigCare's Working Owner model, the BCBS Nebraska partnership, eligibility screening, plan design, and the tradeoffs you should weigh before enrolling.

See If You Qualify for GigCare

Answer a few quick health-screening questions and we'll tell you in seconds whether you potentially qualify for GigCare's Working Owner plan.

    What Is GigCare?

GigCare is marketed as a major-medical health benefit for self-employed people, 1099 contractors, and small business owners who don't have access to employer-sponsored insurance. The plan sponsor is Population Science Management (PSM), a privately held data analytics company. Detego Health LLC, a Texas-based third-party administrator, handles claims, member services, and precertification. The flagship version uses Blue Cross Blue Shield of Nebraska for in-state network access and the BlueCard PPO for nationwide access.

What makes GigCare different from a normal individual policy is the legal wrapper. You are not just buying insurance. You are joining PSM as a "Working Owner" (a preferred shareholder with no voting rights and no liability) and agreeing to complete periodic survey or health-data activities. Because PSM treats you as an owner-participant, you become eligible for the group health plan that PSM sponsors for its Working Owners.

Per public Working Owner materials, the activity cadence is typically once per quarter to once per month, and Working Owners receive guaranteed payments (often around $25 per survey) reported on a year-end Schedule K-1. Missing required activities can terminate Working Owner status and end health benefits.

    Who GigCare Is Designed For

GigCare is built specifically for people who fall through the cracks of the traditional health insurance system - too established for the Marketplace subsidies to help much, but without an employer offering group coverage. Four buyer groups in particular tend to fit:

    Self-Employed Individuals

1099 contractors, freelancers, gig workers, and consultants who don't have access to employer-sponsored insurance and want a real major-medical structure with a national PPO network.

    Early Retirees

Adults who left work before age 65 and need to bridge to Medicare. Unsubsidized Marketplace premiums climb sharply after 50, and Working Owner age-band pricing often comes in significantly lower.

    Small Business Owners

Solo operators and owner-only S-corps that don't qualify for traditional small-group coverage (which generally requires W-2 employees beyond the owner) but want something more structured than an individual ACA policy.

    Households Above the Subsidy Cliff

Families whose income is too high to qualify for meaningful ACA premium tax credits. Without a subsidy, Marketplace premiums can run $1,200-$1,800/month for a couple in their 50s; GigCare can cut that materially.

For a deeper breakdown of which households inside these groups are the strongest fit, see Who GigCare Is For.

    What Plans Does GigCare Offer?

Per the public benefit comparison effective January 1, 2025 and the 2026 rate card, GigCare offers five major-medical plan options. Note that public marketing copy is occasionally inconsistent (one FAQ describes the lineup as "2 PPO & 3 HMO," but the published benefit summaries clearly show PPO and EPO designs). Always verify against the issued schedule of benefits.

    PlanNetworkDeductible (Self/Family)Out-of-Pocket Max
    PPO $1,500PPO + BlueCard$1,500 / $3,000$7,350 / $14,700
    PPO $2,500PPO + BlueCard$2,500 / $5,000$7,350 / $14,700
    EPO $5,000EPO (no out-of-network)$5,000 / $10,000$7,350 / $14,700
    PPO/HSA $5,000PPO/HSA$5,000 / $10,000$6,550 / $13,100
    EPO $7,350EPO (no out-of-network)$7,350 / $14,700$9,200 / $18,400

Office visits on non-HSA designs are typically $25 PCP / $40 specialist. Generic Rx is $10 retail; non-preferred drugs are $105 on PPO designs and not covered on EPO designs. Specialty drugs are explicitly excluded on the current public FAQ, members are referred to manufacturer assistance programs instead. Preventive care follows ACA preventive guidelines at 100% in-network.

    2026 Monthly Contributions

Per GigCare's published 2026 rate sheet. There is no income-based subsidy.

    Where GigCare Wins

For the right household, GigCare delivers genuine advantages that the individual ACA Marketplace can't match - especially on premium cost for unsubsidized buyers and on network depth.

    Lower Premiums for Unsubsidized Buyers

Self-employed households earning above the ACA subsidy cliff often see hundreds of dollars per month in savings - especially in the 50-64 age band where unsubsidized Marketplace premiums climb sharply. The 2026 GigCare rate card shows employee-only premiums starting around $600/month even at older ages.

Real Blue Cross Blue Shield Network

NEtwork Blue in Nebraska and the BlueCard PPO nationwide on PPO designs. The same provider contracts that support major BCBS employer plans, not a rented or limited specialty network.

    True Major-Medical Structure

Includes a deductible, coinsurance, published out-of-pocket maximum, preventive care at 100% in-network, telehealth, and care management. This is real major medical - not a health-sharing ministry, not an indemnity-only product.

    Transparent Benefit Menu

Published benefit comparison summaries and 2026 rate sheets make it relatively easy to evaluate plan design before enrolling. Five plan options (PPO and EPO) let you match deductible and out-of-pocket exposure to your budget.

    Fits the Self-Employed Reality

Designed around 1099 income, variable cash flow, and households without HR departments. Year-round enrollment is possible (no waiting for ACA Open Enrollment if you qualify), and the application process is built for solo buyers.

    Group-Plan Feel for Solo Operators

You carry a BCBS-branded ID card, present it at the doctor's office, and providers file claims directly to Detego Health for adjudication - the same workflow your friends with employer coverage are used to.

How the Working Owner Model Works

    1. Suitability Screen

New primary applicants complete a Milliman suitability questionnaire. This is the first gate. People who don't meet the suitability criteria don't proceed.

    2. Health Disclosure for Dependents

Spouses and dependents complete a separate health-disclosure form covering recent cancer, heart disease, stroke, autoimmune conditions, organ transplant history, current pregnancy, and similar items. A 'yes' to listed conditions typically makes the household ineligible.

    3. Working Owner Onboarding

If the household passes screening, the primary applicant becomes a PSM Working Owner. You receive preferred shares and gain access to a member dashboard for activities and benefits.

    4. Recurring Activities

You complete assigned surveys or health-data activities through the app or dashboard, typically monthly to quarterly. Guaranteed payments (~$25/activity per public materials) are issued and reported on a K-1.

    5. Group Benefit Access

While you are an active Working Owner and current on premiums, you have access to the GigCare medical plan through Detego Health as administrator and BCBS Nebraska / BlueCard for network access.

    6. Ongoing Compliance

If you stop completing activities or fall behind on premiums, Working Owner status (and your health benefits) can terminate. This is a continuing obligation, not a one-time signup.

Powered by Blue Cross Blue Shield

One of the reasons GigCare stands apart from most "alternative" health plans is the network behind it. The flagship GigCare plans use Blue Cross Blue Shield of Nebraska for in-state network access and the BlueCard PPO for nationwide coverage - the same networks used by major employer-sponsored BCBS plans.

    In Nebraska: NEtwork Blue

Access to BCBS Nebraska's broad statewide PPO network, including major hospital systems in Omaha, Lincoln, and across the state. Same provider-contracted rates as standard BCBS group plans.

    Nationwide: BlueCard PPO

When traveling or living outside Nebraska, members access providers through the BlueCard PPO program - the same nationwide network that supports BCBS plans across all 50 states.

This is a meaningful distinction. Many alternative or association-based plans rely on rented or limited networks that look impressive on paper but fall apart at the point of service. GigCare members carry a BCBS-branded ID card and present it at participating providers the same way an employer-plan member would. Claims are administered by Detego Health rather than BCBS directly, but the underlying provider contracts and network rates are real BCBS contracts.

    Important Disclosure

GigCare is not the same as an ACA Marketplace plan. Marketing materials sometimes describe it as "qualified" coverage, but the legal status depends on plan documents not displayed publicly. The Department of Labor rescinded the 2018 Association Health Plan rule in 2024 over working-owner concerns, and state regulators have raised licensing questions about adjacent administrators and similar models. Before enrolling, always request the full Summary Plan Description, the issued Schedule of Benefits, and the Working Owner agreement. As an independent broker, I can help you read those documents and weigh GigCare against your full set of options.

    Frequently Asked Questions

GigCare is a group health benefit offered through Population Science Management (PSM), a data analytics company. To enroll, you become a 'Working Owner' (a preferred shareholder of PSM) and complete periodic surveys or health-data activities. Detego Health acts as the third-party administrator, and Blue Cross Blue Shield of Nebraska provides the underlying network and claims infrastructure for the BCBS-branded version. It is structured more like an employer plan than an individual ACA policy.

No. GigCare is not guaranteed issue. New primary applicants complete a Milliman suitability screen, and spouses and dependents complete a separate health-disclosure questionnaire. People with recent cancer, heart disease, stroke, certain autoimmune conditions, organ failure or transplant history, or current pregnancy are typically ineligible. ACA Marketplace plans, by contrast, must accept everyone regardless of health.

Per the public 2026 monthly contribution sheet, employee-only premiums range roughly from $600/month (EPO $7,350) to $880/month (PPO $1,500) for ages 18 to 29, and from $690 to $1,068/month for ages 55 to 64. Family rates run from about $1,548 to $2,950/month depending on plan and age band. There is no income-based subsidy, unlike ACA Marketplace plans.

Healthy, self-employed or 1099 workers who earn too much for ACA subsidies, are not currently pregnant, have no major chronic conditions, and don't take specialty drugs. For unsubsidized buyers in their 50s and early 60s, the monthly premiums often beat unsubsidized Marketplace plans. For everyone else, an ACA Marketplace Silver plan with subsidies is usually the better protection.
